The following documentation is requested from students who wish to apply:

1.  A fully complete and signed Application
2. $150 Registration and Administrative Fee
3. A signed Records Request Form (if transferring from another school)
4. Unofficial transcripts of grades and standardized test scores (if available)
5. Proof of Identity (copy of birth certificate or copy of passport)

Step Two: Processing

The student will be interviewed by an administrator to review academic performance and standardized test scores from previous schools. La Core Christian Academy does not accept students with discipline problems.

Step Three: Before You Start

Before attendance, the following must be submitted:

  1. Duval County Entrance Physical Examination and Immunization Form (filled out by child’s physician)
    2. Signed Illness Pick-Up Agreement
    3. Signed Student Handbook Acknowledgement
    4. Tuition Payment
